Vital Statistics handles services such as Birth Certificates, Marriage Certificates, Marriage Licences, Death Certificates etc. You can attend our office to order any of the above certificates and more. All services must be accompanied by Government issued photo ID. If you are unable to present ID to order your own birth certificate, you can designate an agent (someone who has known you for 2 years or longer) to come in and present their ID to order your birth certificate on your behalf. Additional fees to have this form commissioned may apply.
